Home care in Canada is more than ten provincial, three territorial and four federally funded programs.  

Home Care in Canada is a harmonized approach to meeting the health and social needs of vulnerable and disadvantaged Canadians wherever they live.  To meet this challenge, we must embrace a synergistic approach across the country to realize a system that is greater than the sum of our individual parts.  

At the 2013 CHCA Home Care Summit, home care leaders from across the country will come together to share experiences, learn from each other and build upon our successes to make home care an integral part of an integrated approach to health and wellness.  

Together we can - make client and family centred care a reality, remove barriers and achieve integration, enable accessible services for all Canadians, discover and share knowledge, support health system sustainability, be accountable and achieve better outcomes.
