Strong, viable connections don't just happen. They need to be understood, properly developed and well maintained.   Skills, knowledge and experience are required when we bring people together to form useful and productive connections and partnerships. 

 

The 2009 CHCA Home Care Summit “Connections” was the venue for achieving these goals.  

 

Over the 2 ½ day Summit, the delegates:

 

·        Connected with People and identified the leadership and frontline core competencies that are needed to implement and sustain integrated models of care.

 

·        Connected with the Community and learned about new approaches to building community capacity, effecting positive change and supporting family caregivers as key partners in care.

 

·        Connected across the Health System by sharing strategies to support transferability of integration models that involve home care and other health care sectors.

 

·        Connected through Information Technology by identifying models and actions that can  advance information technology uptake and expansion in home and community care.
